Public Assistance Disaster Forms
Following a Presidential Disaster Declaration, government entities and eligible non-profit organizations may use the following forms to apply for Public Assistance. Enter the appropriate information and print. Keep a copy for your records and submit the original to the Iowa Homeland Security and Emergency Management Division. The REQUEST FOR PUBLIC ASSISTANCE (RPA), and the three forms in the STATE OF IOWA - APPLICATION DOCUMENTS are mandatory forms.
FEMA - REQUEST FOR PUBLIC ASSISTANCE (RPA)
Usually filled out at the Applicants' Briefing, this is the first form filled out in the Disaster Recovery Process. An RPA is the federal form FEMA recognizes as an official application for Public Assistance funds. Each applicant has thirty days from the day of Presidential Declaration to submit this form otherwise they will not be eligible to receive assistance.

STATE OF IOWA - APPLICATION DOCUMENTS (FEDERAL AND STATE ASSISTANCE) - Excel Spreadsheet or PDF
This spreadsheet consists of three documents: Guidance, State Application, and Assurances and Certifications. These forms are required to receive payment.
The Guidance contains instructions for filling out the Application for Federal and State Assistance.
The State Application is used to apply for assistance by listing the Authorized Representative for the Applicant. An Authorized Representative is the individual who has been given signature authority by a governing body for that particular disaster.
Assurances and Certifications is an agreement signed by the Authorized Representative in order to receive any type of Federal assistance.
Single Audit Requirements for Federal Grants - signed and dated by the Designated Authorized Representative. This signature, on behalf of the applicant, signifies agreement and adherence to all of the terms and conditions set forth therein.
STATE OF IOWA - SUBSTITUTE W-9 (VENDOR UPDATE FORM) - Excel Spreadsheet or PDF
Applicants only need to fill out this form if they have not received payments from the State of Iowa in the last two years. It updates the state's fiscal system with necessary information for the payment process.
